As a reminder, parents MUST login to Skyward, verify their student's information and complete all forms. This process must be completed before the start of the school year.

Linked below at the bottom of this page is the form needed for the affidavit process in both Spanish and English. This form will need to be notarized, a copy of the utility bill/lease of the owner will need to be provided, and a piece of mail from the parent/guardian.
